Medical Document Templates
Over the years I have dealt with the frustrations of trying to communicate my symptoms to doctors and later after not being able to care for myself- Finding a way to make sure I am safe. The documents below are what I created to use.
The Medical Cheat Sheet Template is what I post on a corkboard at the entrance of my home so EMT's can immediately see. It is also posted on my fridge for my caretaker, family or friends that are with me. My Medical Power of Attorneys each have their own copy. I make sure to update it every time a medicine is changed, I have a new doctor, or new condition.

The Medical Symptoms Template helps you organize your symptoms and conditions by body system in chronological order. If you aren't sure which system it goes in, that's ok, just give your best guess. The important thing is this helps them see the picture.

The Family Tree Conditions and Symptoms Template helps you organize your families conditions and symptoms by body system and familial side. This is especially important for genetic disorder considerations.

A lot of these are questions you will be asked at new patient appointments so you can have them already typed up and printed to the appointment so you don't have to type- which takes a lot of SPOONS.
